Failure to read and follow instructions furnished in this instruction manual may result in serious patient injury. Device must not be used if it appears to be damaged or functioning incorrectly. Ensure the mating sunbursts teeth of the Swivel Adaptor and the Stationary Horseshoe Faceplate are the same size and have properly engaged (meshed) when fully tightened. Failure to do so may damage teeth, compromise stability, and result in serious patient injury. Do not position the slide Horseshoe Faceplate past the end of the stationary horseshoe faceplate slide bar. Doing so may result in serious patient injury. Horseshoe faceplates should be adjusted prior to patient positioning. Make certain to always support the patient’s head when adjusting the horseshoe faceplates or unlocking any connection of this device. Failure to do so may result in serious injury. Failure to properly position the patient and secure all connections on this device may result in serious patient injury. Pulley rod may be used for traction of up to 40 lbs. Exceeding 40 lbs. of traction load may result in device failure and serious patient injury. Device must not be used if it has been dropped or mishandled. Doing so may result in serious patient injury. Do not steam sterilize any component of this. Doing so may damage device and result in serious patient injury. Failure to properly position the patient and secure all connections on the device may result in serious patient injury.

The adjustment knob is permanently attached to the slide horseshoe faceplate, use of excessive force in an attempt to remove the adjustment knob from the slide horseshoe faceplate can result in damaged components. Do not use an alcohol solution on the Gel Pads, immerse the Gel Pads in liquid, or expose the Gel Pads to heat via steam sterilization or any other means. Refer to Mayfield Radiolucent Headrest System instruction manual (REF 4-51-A-2004) for the correct usage of the Mayfield Radiolucent Base Unit prior to using this device. If applicable, refer to the Mayfield Radiolucent Tri-Star Swivel Adaptor instruction manual (REF 4-51-A-1111) prior to using this device.
